Derive the full sixteen-octet channel identifier for a key.
The two-octet channel_id is its prefix and is what travels on the wire;
this is the width at which two channels can be told apart, which is what
naming one to a device takes. It is what the channel-key tables report
for each key they hold, what PROP_HOST_MUTED_CHANNELS carries, and
what CMD_ANNOUNCE names a channel by.
